For patients who would like to schedule IV Ozone Therapy, PRP, Trigger Point & Joint Injections, Hair Restoration, and Musculoskeletal Consults, please complete the Procedure Intake Form. Currently, we offer these modalities on Tuesdays, Thursdays, and Fridays and there is no wait list.

For patients who would like to be seen for Functional Medicine, Family Practice, or Membership, we are grateful that you would like to join The Functional Medicine Center. We kindly ask that you complete the medical intake form (linked below) to get the process started.

Interested patients are welcome to schedule a Discovery Call with Lauren Light, our onboarding specialist & billing manager by texting or calling the main phone number 919-578-2323.

The purpose of the Discovery Call is so that you can have your questions answered about our Practice. You may submit your medical intake form at any point before or after the Discovery Call for our providers to review to assure we can help.

This is a unique onboarding practice that differs from other practices. We want patients to feel comfortable with us before they spend any money. We also want to be sure that we can help each new patient that we onboard.

Please allow us 7-10 days to review your intake form. If we decide that we are a mutually good fit, we will ask that you complete a few more forms – the communication policy, the privacy policy and the financial agreement via our patient portal. Then, we will call you to schedule your first appointment. Because of our commitment to provide the best possible care and service to our current patients, we may not be able to schedule your initial visit for several weeks from the time you complete your intake form. We will let you know this as honestly and transparently as possible after we review your forms.
